This is a new service to allow patients to monitor their BP at home and have the readings automatically sent to the prctice for review at agreed timescales. Monitoring BPs over a period of time is much more accurate and safer as it helps avoid mis diagnosis of hypertension (raised BP), overtreatment (which can lead to falls and injury) and alerts patients to contact the practice between reviews if there's a significant change in BP.
